CARE IN THE COMMUNITY

Our Regulated Services can be a real help when you want to stay within your own home environment. If your recovering from an Accident, Injury or Operation, need Medication Assistance, Personal Care, or feeling overwhelmed by all the household chores or just want some help each day, we can take the struggle out of living at home and support you to remain independent within your own home.

Our caregivers can give you a hand with everyday jobs around the home: Personal Care, Medication, Health Care, Oral Care, Care to manage your household chores, accessing Community Facilities and even going shopping. Whatever you need,’ we always deliver our Service with a Smile, Empathy, and Consideration, remembering your Right to Respect every time.

SHORT & LONG-TERM CARE

We provide Support to our citizens that wish to remain in their home and live independently we can offer a tailored package that will suit your needs. Our office team will come and meet with you to discuss what support you need and complete a Care Plan together to put the package into place, this can be either be for a short duration or on a permanent basis. Our calls start from 30 minutes depending on your needs, we can also offer single or double caregivers, depending on the support you need.

LIVE IN CARE

If you still would like to remain in your home and continue to be part of your community, but require assistance on a 24 hour basis, we are able to provide a permanent live in carer who will stay at your home and provide you with support for you to stay independently, therefore we will work with you to match caregivers who will live in and stay at your home on a permanent basis, all our caregivers are vetted, trained before they commence working for us and are there to make life easier for you and to help you achieve it. We review the Service carefully with you to ensure that it’s working the way you want and agree any necessary adjustments to your care plan, throughout the service we provide.

RESPITE CARE

Do you require any help after you have had an operation or are the people that Support you at home going on holiday, therefore our team will be able to put in a short-term package with yourselves that will support you with the care you require during this short period of time. Our dedicated team will work with you to put in the best support for short time period.

EIDERLY CARE

Elderly Care or Domiciliary Care as it is known, is provided to people who still live in their own homes but require additional Support with Activities, including household tasks,Personal Care and any other activity that allows people to maintain both their Independence and Quality of Life.

We can offer a short-term package of Care to help with Support after an Operation or Injury after time in hospital or companionship through a bereavement, as well as offer a more long-term solution in-order for you to stay at home or to provide respite for you as a main carer.

We are able to provide packages of Care starting from a 30-minute call and then increasing depending on the need of the individual. Our Services are available: Privately, Local Authority Funding, continuing Health Care Funding, Personal Budgets and Clinical Commissioning Groups.

CHILDREN SERVICES

We provide trained staff to support Children and Young People in their own home. Support provided includes help with day-to-day Activities including Washing, Dressing and Eating, Support to get ready for School or College.

Care Avenues are working with Children and families in the community to support parents or care givers caring with Complex Needs and life-threatening conditions Autism and Epilepsy.

These Children may be Peg Fed or have a Tracheotomy or need suctioning or all three. Autism brings different challenges such as Challenging Behaviour or Epilepsy which can be life threatening if not monitored.

We aim to provide bespoke Services that are centric to the individual needs of the Child following Key Legislation, Safeguarding Principles and Child Protection Measures.

We provide Specialist Training to all our Support Workers including Paediatric First Aid, Peg Feeding, Tracheotomy Care, Oxygen Monitoring, Suctioning, Autism Awareness and Epilepsy including all Mandatory Training and Medication Administration.

ADULT SERVICES

We Provide Companionship, Comfort & Compassionate Care and Support for those needing Extra Care in their own Home Environments: Short & Long Term, Live-In Care, Dementia Care, Care for Couples, Respite Care.

WHAT IS DEMENTIA?

Dementia is a general term for Loss of Memory, Language, Problem-Solving and other thinking abilities that are severe enough to interfere with daily life. Alzheimer’s Disease is the most common cause of Dementia. We understand that you might want to stay in your home to stay in familiar surroundings and maintain your daily routines, that otherwise would not be possible if you were in Residential Care. Therefore, our team will be able to discuss with you and your family what type of support you require to enable you to continue with the continuity of life within your community.

We cater for Service Users with Dementia, all our caregivers are well trained in supporting those living with Dementia and our staff achieve the RQF LEVEL 2.

MENTAL HEALTH

Mental Health includes our Emotional, Psychological, and Social Well-Being. It affects how we think, feel, and act. It also helps determine how we handle Stress, relate to others, and make choices. Mental Health is important at every stage of life, from Childhood and adolescence through Adulthood.

Over the course of your life, if you experience Mental Health problems, you’re thinking, mood, and behaviour could be affected. Many Factors contribute to Mental Health problems, including:

  • Biological Factors, such as Genes or Brain Chemistry
  • Life Experiences, such as Trauma or Abuse
  • Family History of Mental Health problems
