A care coordinator visits at home to talk through routines, health needs, preferences and what a good day looks like.
We write a care plan built specifically around that person, agreed together with them and their family before anything begins.
We match a regular carer, or a small consistent team, based on personality, interests and needs — not just availability.
We check in regularly and adjust the care plan as things change, so support keeps pace with real life.
No two routes into care look the same. One family needs an hour each morning; another needs someone in the house full-time. We built Care Avenues to hold all of those routes at once, without forcing anyone down a path that doesn't fit.

Every carer is DBS-checked, trained to Care Certificate standard and regularly supervised.
